The Lewin Group, under contract to the Agency for Healthcare Research and Quality (AHRQ), developed "Designing and Implementing Medicaid Disease and Care Management Programs: A User's Guide." The Guide, reflecting the experiences of 13 Medicaid care management programs, is designed to be a resource for decision-makers involved with designing and implementing care management programs.  The Guide addresses planning a care management program, engaging stakeholders in a care management program, selecting and targeting populations, selecting care management interventions, selecting a care management program model, operating a care management program, measuring value in care management programs, and the care management evidence base.  Please click here to access the Guide on the AHRQ website. 

On October 5th, the Guide will be highlighted at the NASHP Annual Meeting. The full day pre-conference, sponsored by AHRQ, Medicaid Care Management: Lessons Learned and Future Trends, will address successful programs in Illinois and New Hampshire; strategies used in Virginia and Texas to plan new programs; engaging providers in North Carolina and collaboration with medical homes in Washington; and trends in care management such as focusing on co-morbid conditions and predictive modeling.  There is no registration fee for the pre-conference, but registration is limited.  To register, please visit the NASHP website.
